Decoquinate/Levamisole HCl/Elevate is a compounded formulation containing Decoquinate, Levamisole HCl, Elevate, prepared by Mixlab for veterinary use. Available by prescription.

Powder medications are meant to be mixed into food or treats, not given on their own (since most pets won’t lick up plain powder). The key is to mix it into something tasty so your pet eats the entire dose.

How to Give It:

  1. Measure the dose using the included scoop or follow the instructions. If you’re unsure how much to give, ask your vet or pharmacist.
  2. Mix it with a small amount of soft food or treat—like wet food, baby food (meat flavor), peanut butter (xylitol-free), yogurt, or cheese. Use just enough to hide the powder so your pet finishes it all in one go.
  3. Stir it well to blend the powder in evenly. Add a splash of water if needed to make it smoother.
  4. Offer it when your pet is a little hungry, and watch to make sure they eat the whole thing.
  5. If your pet refuses, try something else they love next time. For cats, you can also try smearing the paste on their paw to lick off (only if safe and comfortable).
  6. For large animals, mix the powder into a small portion of feed with water or molasses, then follow with the rest of their food once they’ve eaten the medicated part.
  7. Store the powder in a cool, dry place with the lid closed tightly.

Never give powder straight into your pet’s mouth—it can be dangerous if inhaled.

This compounded formulation combines active ingredients from different drug classes: decoquinate, an antiprotozoal; levamisole, an immunomodulator; and a vitamin E supplement. In compounding practice, clinicians may consider a multi-ingredient preparation like this to address more than one aspect of a patient's physiology at once. Your veterinarian determines whether it is appropriate based on your animal's diagnostic findings.

At a pharmacological level, the components of this formulation act through distinct but integrated pathways. Decoquinate acts as an antiprotozoal by disrupting the mitochondrial electron transport system within the parasite, halting its energy production. Levamisole HCl serves a dual role: it functions as an anthelmintic by causing spastic paralysis in susceptible nematodes and is studied for its immunomodulatory effects, specifically its ability to support T-cell function and macrophage activity. Elevate (natural Vitamin E) is included as a potent antioxidant that supports neurological health by protecting cell membranes from oxidative stress, which is often a point of clinical focus during the management of central nervous system inflammation. Your veterinarian will determine the appropriate application of this combination based on your horse's weight, neurological status and specific diagnostic findings.

This description reflects the general pharmacology of the active ingredient's drug class; your veterinarian determines whether this approach is appropriate for your animal.

Commonly observed effects of decoquinate/levamisole HCl/Elevate administration include mild gastrointestinal changes such as loose manure or a temporary decrease in appetite. Because this medication targets internal parasites, owners may observe a transient exacerbation of neurological signs—often referred to as a "treatment crisis"—as the body responds to the targeted protozoal life stages. Protective measures should be taken to ensure the horse remains in a safe environment during the initial phase of therapy. If you observe significant stumbling, persistent diarrhea or signs of physical distress, contact your veterinarian promptly.

This medication is typically used with extra caution or avoided in debilitated animals or those with significant kidney or liver impairment. It is also carefully evaluated in breeding, pregnant or lactating mares, as the safety profile for developing foals is a point of professional consideration. Before starting a protocol involving this combination, ensure your veterinarian has a complete history of all other prescriptions and supplements your horse is receiving, as levamisole can interact with other compounds that affect the nervous system. Your veterinarian is the right person to design a comprehensive monitoring plan and provide guidance on the necessary duration of therapy to ensure the management approach remains suitable for your horse's individual health status.

Do I need a prescription to get decoquinate/levamisole HCl/Elevate for my pet?

Yes. Decoquinate/levamisole HCl/Elevate is a combination of prescription medications and can only be dispensed by a licensed pharmacy upon receipt of a valid authorization from a licensed veterinarian. Mixlab verifies all authorizations before any medication is prepared or shipped to your stable.

Can decoquinate/levamisole HCl/Elevate be compounded for my pet?

Yes. Since this specific three-part combination is not available as a single commercial product, Mixlab compounds it into specialized formats—such as flavored oral pastes or powders—when a veterinarian determines that a customized preparation better suits an individual patient's needs. The decision between a commercial and compounded product rests with the prescribing veterinarian.

Why is Elevate Vitamin E included in this combination?

Veterinarians discuss the inclusion of Elevate (natural Vitamin E) because it provides potent antioxidant support for the nervous system during periods of parasitic stress. It is specifically studied for its water-solubility, which allows it to reach the central nervous system than standard synthetic Vitamin E supplements.

What is the "treatment crisis" sometimes seen with this medication?

As the medication acts on the protozoal parasites within the nervous system, the resulting inflammatory response can occasionally lead to a temporary worsening of coordination or weakness. Your veterinarian will provide guidance on how to monitor your horse during this period and may adjust the management plan based on your horse's individual response.
