Numelvi is an FDA-approved oral tablet containing atinvicitinib, for veterinary use in dogs. Available by prescription.
Veterinarians consider Numelvi for dogs 6 months of age and older presented with pruritus (itching) associated with allergic dermatitis. It is administered once daily, at or around mealtime, as part of a plan for managing the underlying allergic condition.
Numelvi's active ingredient, atinvicitinib, is a Janus kinase (JAK) inhibitor with selectivity toward the JAK1 enzyme, which is involved in the signaling of pruritogenic and pro-inflammatory cytokines. By inhibiting JAK1, atinvicitinib interrupts this signaling pathway, which is understood to reduce the transmission of itch and inflammatory signals. It is not a corticosteroid or an antihistamine.

In field studies conducted over 28 days, the most commonly reported effects associated with Numelvi included vomiting or nausea (6.9%), otitis externa (6.3%), and hematuria (4.9%). Laboratory monitoring identified changes such as decreased white blood cell counts (leukopenia and neutropenia), elevated liver enzymes, and elevated cholesterol in some dogs; your veterinarian may recommend periodic bloodwork during treatment. Numelvi's effects and safety profile have not been evaluated beyond 28 days of continuous use in field studies.
Because Numelvi modulates immune system signaling, dogs receiving it may have an increased susceptibility to opportunistic infections, including demodicosis and interdigital furunculosis; your veterinarian will want to monitor for signs of infection during treatment. New neoplastic conditions, both benign and malignant, have been reported with other medications in the JAK inhibitor class, so your veterinarian may discuss this as part of ongoing monitoring. Numelvi is not used in dogs younger than 6 months of age or those with a serious infection, and caution is advised in dogs with a history of recurrent demodicosis. Its use has not been evaluated in breeding, pregnant, or lactating dogs, or in combination with glucocorticoids, cyclosporine, or other systemic immunosuppressive medications — let your veterinarian know about any other medications your dog is taking.
