Diltiazem is a calcium channel blocker that relaxes blood vessels and slows certain electrical activity in the heart; the extended-release formulation is intended for once-daily dosing. On the FDA-approved human label, it is indicated for hypertension and certain heart rhythm conditions.

In veterinary medicine, extended-release diltiazem has been studied for once-daily use in cats, most often to help manage hypertrophic cardiomyopathy, and is used in dogs for certain arrhythmias or hypertension. Extended-release products are not interchangeable between brands or formulations — do not switch products or crush/split this medication without your veterinarian's specific direction, since absorption can vary between different extended-release diltiazem formulations. Commonly reported effects include decreased appetite, lethargy, and low blood pressure; slow heart rate has also been reported. Your veterinarian will likely recommend periodic monitoring, which may include blood pressure checks or echocardiograms.
